#56 summary intro for lost.xml

An introductory paragraph for logic and approach in
dealing with lost text is needed. This paragraph (or
two) should be placed at the top of lost.xml, before
the inclusions for the subsections. Certainty should be
addressed, as should the meaning of the three loci that
we recognize in this context:

* #gi
* extent
* unit
