I've committed my changes to pvAccessCPP-md.
I haven't yet done pointer to shared_pointer replaces.
It would not compile also, since some changes are needed in pvData (i.e.
pointer typedefs).

What I want you to do is to check
pvAccessCPP-md/pvAccessApp/client/pvAccess.h if its OK for you.
