#172 C examples in docs for SSFF format are broken

The example C code in the documentation for the SSFF file format (here currently: http://emu.sourceforge.net/manual/x1572.html\) are broken. There's some incorrect formatting, but also references to incorrect function names and headers.

    I rewrote the read/write example, correcting the compilation problems. I also changed the read example a bit to read the same file that was created in the write example.

    I'll attach a patch for the emu-doc module. However, I noticed in the latest emudoc the SSFF content is commented out of manual.xml and chapters/fileformats.xml. So, if the SSFF content is intended to be removed, the patch is useless.

    But since the SSFF documentation content is still on the emu website, it could be updated with the working examples.
