#124 docs: add an index to the LinuxCNC_Manual_Pages pdf document

Currently the PDF doesn't have a table of contents, so you can't easily skip to the manpage you want.

I'm not sure if anyone reads manpages in pdf format, but this feature would also be useful for folks who like to print out documentation.

Which I generally do about bi-annually as a paper copy can go anyplace you
can find space to lay it out.

A TOC would be a welcome addition here at the coyote.den. Might make me
waste another tree. ;-)
