Sounds like a dragon NaturallySpeaking version 7 bug.  There have been 
numerous complaints of this nature about version 7 with non Microsoft 
programs.   Is that the speech program you're using?  Have you implemented 
the recent version 7.2 patch?  I am curious just how much the patch reduces 
this kind of bugs.
