Even though the checkbox has been removed, implicitly php_azure.dll should always be added to project within:
\ServiceDefinition.csx\roles\WebRole\approot\php\ext

The latest release 1.1.0-dev (16t-Dec-2009) of
http://phpazurecontrib.codeplex.com/Release/ProjectReleases.aspx?ReleaseId=37366#DownloadId=97394

works well with azure sdk 1.0.
