#15 Correct PMT fixing
The current code for PMT fixing (which is used for placing streams in PMT in the right order - first video, then preferred audio, then rest of the audio and then the subtitles) works only if PMT fits a single TS packet. If it doesn't, the program crashes. The current workaround (using "DontFixPMT=1") disables the functionality above.
