[Doxygen-users] Stylesheets / Headers / Footers

I tried, for the first time, to customize these options.

1) Although I specified  "HTML_STYLESHEET = stylesheet.css", the generated
   HTML are referencing doxygen.css.  A bug, no?

2) Is there a way to make the header/footer inclusion dynamic?  This way I
   can change the header/footer and see the results without having to do a
   complete doxygen reprocessing of my code.  Right now the text of the
   header/footer are hard-coded into each file.

This is with doxygen 1.2.10.
