Updated add-in release (0.4.1)

I've released a patched version of the Visual Studio 2005 add-in. It includes a number of changes to properly support localised versions of Visual Studio 2005.

Big thanks to Jungjin Kim for helping me test the localisation code.

I haven't had time to look at the change requests yet, but please keep suggestions and bug reports coming! I don't always have time to test everything in details, so I really appreciate any feedback to improve the release quality.

Sorry for the long delay in getting this one out the door (my job is a nightmare these days). ..
