Compatibility patch for new GCC

The GCC of version 4.6.1 does not allow the usage of iostream.h and fstream.h in include directives, but uses iostream and fstream without the .h ending. The attached patch from the src/ directory fixes this problem making DOC++ compilable with recent a GCC (such 4.6.1 as in Ubuntu 11.10 and perhaps newer).
