#1 HTML Index of all documented items

I cannot seem to get doc++ to create an HTML index of
all documented items when I use the /*@Include: ... */
directive. The table of contents feature *almost*
works, but the functions are not necessarily
alphabetized, even when I use the --sort option. It
would be very nice to be able to create one page that
had all implicitly and explicitly @named items, in
alphabetical order, with links to them.
