#11 paragraphs do not have correct indent after nested lists

Because the .RS/.RE (as I've found out)
don't restore the indents in a stack.

Two possible solutions (not mutually exclusive):

1. have db2x_manxml workaround this brokenness
of the .RS/.RE macros

2. Change XSLT stylesheets so they don't use
the .RS/.RE macros at all.
