#45 xref format subelement

Request: Add subelements to let the user
specify the appropriate format for the
context of the text generated by the xref.

(This request for a DTD change parallels
the short-term request for an XSL feature
in 451011.)

Rationale: Different text should be generated
in different contexts. The full, quoted title
is appropriate in some contexts but the page
number is appropriate in others.

Use Case: Space is limited in the context
(for instance, in a table) or multiple targets
are being listed, so the full title is not necessary.

Possible Approach: Allow optional content
within an xref that includes a format
subelement. The format subelement can
contain a list of fields for the generated text:

<xref linkend="...">
<field name="label"/>
<field name="quotedtitle"/>

    Adding formatting information to the xref element would
    introduce presentational information too directly into the
    semantic markup.

    Suggested workarounds: use the role attribute to identify
    the presentational behavior desired, or use ID attributes on
    the xref elements and construct your stylesheet to format
    specific ID'd xrefs differently.
