Appendix, Chapter, and Preface refer to
"(%nav.class)*", which
misses a semicolon.

ubiq.{in,ex}clusion have a similar problem with
"(%ubiq.mix)"

    The semicolons aren't technically required in SGML. But the
    problemw ill go away in 5.0 since the base DTD will be XML.
