#240 Rename firstname and surname

I've now received (at least) two separate suggestions that we rename firstname to givenname and surname to familyname. Most recently, here:

Now is the time, I suppose.

    Replacing firstname with givenname seems like a clear improvement. The rationale for that change is maybe obvious enough that I
    really doubt anybody can or will reasonably object to it.

    But it doesn't seem obvious to me at least that replacing surname with familyname would be a clear improvement.

    The TC concluded that this is a feature request not a bug and will not be addressed before 5.1.

    Making this change, which is backwards incompatible, would now be quite difficult. The TC decided that a reasonable compromise would be to allow givenname as an *alternative* to firstname. A compelling argument was made that surname *is* a reasonable tag name, but firstname is very western centric.

    This is a backwards compatible change so can be made in the next release.
