The template formal.object.heading uses the parameter placement to decide whether it should use a keep-with-next.within-column or a keep-with-previous.within-column attribute to keep the formal object with its title.
This template is called from formal.object, but the caller doesn't sets the placement parameter. As a result the title isn't always kept with the object when the placement is 'after'. This patch corrects this bug:
