#177 Optimizations for html stylesheets
Using xsltproc's --profile option, I've found some bottlenecks in the stylesheets. This patch improves the time required to generate Boost's html documentation by a factor of 2-3. (I've overridden generate.html.title separately, to reduce the number of calls to gentext.template).
