#167 Added support for @startingnumber in docbook stylesheets

In the docbook 5 spec, the attribute startingnumber has been added to ordered lists however there is currently no code to handle that in the docbook XSL stylesheets. This patch will handle that. This is my first submission, I hope I did everything right.

    I did not apply your patches, but there was a bug that has been fixed and @startingnumber should work now.
