After rendering the fo file with FOP or XEP they get the same indent width as level 2 entries. I think the reason for this is the missing start-indent attribute of the corresponding fo:block element (id="toc...idFOO"). When setting it to "0pt" everything seems to be ok. If I set the draft.mode param to 'yes' the toc is just fine. What is the fix for this? I haven't been able to figure it out myself or by looking around here.
