For HTML output, the solution is CSS. For FO output, you will need a customization layer for now. For help with that, post to docbook-apps. If nobody else responds, I will. To facilitate creation of an FO customization to handle it, I guess an epigraph.properties attribute-set could be added to the FO stylesheets.
