#95 slides: Use parameter for file extension

I'm using MathML in Docbook slides via a customization
layer.

In order for Mozilla to render MathML the files need to
have either .xhtml or .xml extension. The files produced
by 3.0b1 have .html extension.

In the file slides-common.xsl I've changed

<xsl:text>.html</xsl:text>
to
<xsl:value-of select="$html.ext"/>

where $html.ext$ is a parameter defined in the main
Docbook stylesheets.

I set
<xsl:param name="html.ext" select="'.xhtml'"/>
<xsl:param name="titlefoil.html" select="'index.xhtml'"/
<xsl:param name="toc.html" select="'toc.xhtml'"/>

Seems to work ok (Saxon 6.5.2, latest releases of
Docbook) ...

(I also made other changes to get xhtml output though).
