#413 Wrap olink sitemap <ttl> and <xreftext> in CDATA section

Origin of this request is a bug listed at
http://lists.oasis-open.org/archives/docbook-apps/200808/msg00065.html

The request is to wrap all content of all <ttl> and <xreftext> tags in the target db in an CDATA section.

This has two advantages:
- named entities will not be parsed while processing the sitemap (e.g. while copying the content of <ttl> to the output document). It may lead to an error when XInclude is used to assemble the sitemap document because XInclude does not know the named entities (see link to discussion above).

- xml tags will not be parsed while processing the sitemap. This has the advantage that even non-xhtml text can be processed by the xsl stylesheets. Otherwise something like <ttl>"My Super<wbr>califragilistic<wbr>expiali<wbr>docious Title"</ttl> might lead to an error.
