The following is a minor bug: Processing a refentry
source with HTML stylesheets leads to headers 'Name'
and 'Synopsis'. In accordance to the Linux Man Page
Howto and the manpage stylesheets, these headers should
be in uppercase letters: 'NAME' and 'SYNOPSIS'.

HTML ain't man pages :) The HTML stylesheets for handling
Refentry do not necessarily attempt to adhere to guidelines
about formatting of man pages. They are separate formats,
with separate conventions. You can get uppercase in HTML
output using the CSS text-transform property. But I guess
we can add an option to output uppercase for Refsect1
headings in HTML, though the default will remain for them
to retain the same case as they have in the source.

This is a style issue. We have moved away from hard-coding styles or even from providing stylesheet options for tuning styling. The proper place to do it is with CSS.
