#140 html/docbook.xsl should generate <!DOCTYPE>

I would like to be able to pass the w3.org HTML validation tests on my html output.
Currently using xsltproc and xml and xsl, I get no <!DOCTYPE> declaration on output, so I fail
the validation.

Please add a parameter to force the xslt processor to output a <!DOCTYPE> declaration.

If I use xhtml/docbook.xsl, I do get the DOCTYPE declaration. But since I have to run on a very
large range of browers, the project leaders have decreed "No xhtml".

    Use xsl:output doctype-*or the chunker.output.doctype-*
    parameter.
