Kakadu version 6.3, which we are currently using, requires that a filename with
a proper image extension be supplied to kdu_expand -- it will not write to
/dev/stdout, but aborts with an error. I have a patch against the 1.1 distro
that works around this problem by creating a temporary file with the right
extension for kdu_expand to write into. The extension must be ".ppm" for color
images and ".pgm" for grayscale images.
