Two requests here:
Fix a bug in Word2DITA footnote/endnote processing whereby:
a. Endnotes are wrapped in a "ph" element, whereas footnotes are not (see attached topic_2.dita line 258 and line 86). Presumably the correct markup structure is what currently happens to footnotes so that endnotes need to be fixed to match.
b. An empty "ph" element occurs in both footnotes/endnote paragraphs. (See, again, the "p" elements and their content on the lines referenced above.)
Modify the D4P code that processes Word footnotes/endnotes so that @outputclass is always added to the "fn" element with the appropriate value "footnote" or "endnote":
<fn outputclass="footnote"><p>Footnote content here</p></fn>
<fn outputclass="endnote"><p>Endnote content here</p></fn>
This will make processing of footnotes/endnotes easier because you will not have to indicate whether the note is a footnote or an endnote via the paragraphs inside it and also allow paragraphs inside footnotes/endnotes to have aribitrary styles.

Need to update code in wordml2simple that handles footnotes and endnotes and then code in simple2dita that generates the DITA fn elements.
