#62 Handle grouped styles in InCopy output

When styles are grouped in InDesign, the InCopy file must also define the styles in groups. This means the style handling needs to do the following:

1. Translate ":" in style names to group separators. The value put in the ICML file should be {groupname}%3a{style name}

2. Generate style group definitions for grouped styles during style generation
