HTML5 plugin does not seem to be handling images with same name but in different directories.
When source dita document has multiple images with same name but in different directories, the resulting image directory has only one image and the resulting HTML files refer to this single image.
E.g. Consider a dita document having:
<image href="../images/myimage.png"/>
and later
<image href="../images/mydir/myimage.png"/>
The resulting image directory shall have only myimages.png file and the resulting HTML has: for BOTH the image elements.
I believe in the copy-graphics.xml, I see something being done but not with correct outcome:

<fileset dir="/mysrcpath/image">
<include name="myimage.png"/>
</fileset>
</copy>
<copy toFile="/mybuildpath/images/toolbarPanel-2.png" overwrite="yes">

<include name="myimage.png"/>
</fileset>
</copy>
