#128 Make Dita For Publishers work with DITA OT 1.8

Replace in the plugin build files constructs like this:

       <fileset dir="${dita.resource.dir}" includes="*.css"/>

with:

       <fileset dir="${dita.plugin.org.dita.xhtml.dir}/resource" includes="*.css"/>

In the called <xslt> tasks add a reference to the main dita catalog:

       <xmlcatalog refid="dita.catalog" />

    All these build files:

    build_transtype-html2_template.xml
    build_transtype-kindle_template.xml
    build_transtype-epub_template.xml

    should refer to the main catalog:

      <xmlcatalog refid="dita.catalog" />
    

    when the main xsl task is run.

    • the XSLT stylesheet "map2html2Impl.xsl" has a reference like:

      <xsl:import href="plugin:org.dita.base:xsl/dita2xhtml.xsl"/>
      

    which should be changed to:

       <xsl:import href="plugin:org.dita.xhtml:xsl/dita2xhtml.xsl"/>
