[Dia2code-general] patch for horizontal mapping for the sql code generator

I wrote a patch that implements horizontal mapping as described here:

A sample dia file can be found at

generated sql: http://publish.fh-sbg.ac.at/~dweller/horizontal.sql

This is my first contribution to an open source project, so any and all
comments regarding my submission are very welcome.
