I ended up using Cyrus SASL for outgoing authentication and Dovecot SASL (Postfix 2.3 has support for this and is included in the latest CVS snapshot) for incoming SMTP-AUTH with TLS. I did some changes to the CVS repository, I'm uploading you the latest compile to ftp://ftp.devil-linux.org/pub/devel/testing/ right now.
