#92 'make' option

This thing is awesome, but it would be nice if it could detect which files have not changed since the last doc build and NOT rebuild for those files, much like 'make'.

We're stumbling on this as for our data access library it generates 1600+ new files on every build (even if only one file was actually changed), which go into subversion, and consequently make the next update for the developers on the other side of the internet big and slow.
