It would be great if DEP4E could provide this as an additional output format so that the user then only has to run the resultant output through the Microsoft supplied CHM compile option.

OK I found this link on Sagehill which seems to suggest that creating Windows help is quite doable with a DocBook XSL - note this comment:
Process your DocBook XML document with the DocBook htmlhelp.xsl stylesheet.
