#21 Report: Include attempted usernames

When sending a report, it'd be really handy to see why the ip was blocked (root vs. invalid vs. too many tries) and if the list of users is less than (for instance) 3 then show them too.

This would let me be sure I wasn't blocking one of my particularly fat fingered users and could contact them. Or if they thought they were logging into a different machine (same account name, different password), etc.

It would let me be a more proactive admin, which is good.
