#9 Add BeneficiaryName to the CSS forms, and Data entry screen

The CSS needs to add a new field to the data entry form
and the CSS survey form. This field should be filled
with the BeneficiaryName which is differ than the
subscriber name.
I propose to add BeneficiaryName varchar(64) to the
database, and adjust the application accordingly.

To which table should the field be added?
I think just Beneficiary as the fieldname
is enough, no need for BeneficiaryName.
