[Ctool-develop] little fixes

hi there,

trying to compile the ctool code with gcc 3.2, I run into two
minor issues with missing 'std::' prefixes and a redundant
default initializer. The patches are appended. (the diff is
against the latest release (2.11), as sf.net's cvs service
isn't working properly (sigh).
