I didn't really test it yet. If you do tests I'll be interested.
With recent version as explained in issue 81 comment 47, it should not be really linked to the number of accounts anymore. At least, it should affect less the battery consumption cause all keep alive are done at the same time.
Something that could drain much more on one account is the fact that the sip provider is verbose. For example some sip providers send a lot of NOTIFY. In this case, as it wakes up the device for each notify, obviously it will drain more battery.
