This appears to be an issue with ICE on at least some versions of Asterisk. I found that the same conditions happen on Asterisk 11.7 (Ubuntu 14.04 prepackaged), even with icesupport=yes configured.

For the purposes of the pbxes.org wizard, it should probably enable "Custom ICE configuration" for the account, with "Enable ICE" disabled, to prevent the use of ICE with the account even if it's enabled globally.

(I've since disabled ICE globally as it's not really needed with the systems I use, but it's good to note this exception case to assist others.)
