[Cryptotw-usage] Problems with CryptoTW 1.0.1.x

I've had a few reports from people about problems with the 1.0.1.x code and
Microsoft development environments accessing the encrypt functions.  I'd
like to consolidate the bug reports and get to the root cause(s).   If you
are using CryptoTW and VB and are having problems, please detail them here.
