#25 ditch add fault form

I'm thinking maybe we ditch the specify geometry for a new fault segment. It seems to still not work quite right. I put in lat/lon start and end, but x/y didn't get updated until I entered a new lat on the fault components form. Perhaps instead, we have that form with an add fault (new line) button. Then you should have the same form that already seems to work well ready to go.
