Is there a way to modify VisVim to make the Visual Studio addin bring up the
Cream version instead of the standard gvim?

I've never used Visual Studio myself so I can't say. Generally, if you
can pass arguments to Vim, you can specify the Cream startup with:
gvim -U NONE -u "\$VIMRUNTIME/cream/creamrc"
