#9 connect handler

Perform an HTTP request to connect to a remote or local
resource. The content of the element becomes the data
content of a PUT (write) or POST
(append) request. The result is the document returned
from the request, as a <document> element.

If a <URL> and/or <headers> element appear before any
nonblank content, they are used for the connection.

Note: only file URLs can be implemented until cPIA is
integrated with Apache.
