I posted this to the main website, but thought this might be a better
forum. Forgive me if this has already been discussed before.
I use the realvnc (3.3.6) based servers at home and at work. I use a
hacked version of OSXvnc-server so that my 3.3.6 windows client can
send mouse events back, so since I just switched to CotVNC, this is the
one thing I needed.
This patch modifies rfbproto.h to include the scroll wheel buttons.
It modifies RFBView.m to respond to scroll wheel events (BTW the
commented out code already there has the buttons backwards).
It modifies RFBConnection.m to always send out the event for the scroll
wheel (since, unlike mouse buttons, there is no "up" event).
P.S. These probably should be added to rfbproto.h (they are there in
#define rfbWheelUpMask rfbButton4Mask
#define rfbWheelDownMask rfbButton5Mask

The BEST way (IMHO) would be to submit a bug and attach the patch.
Never mind. If Jason doesn't nail this first (unlikely), I'll go ahead
and review and roll it tomorrow.
