#13 Generate valid HRC-REF

The patch makes generation process output valid DocBook and valid XHTML.

- output paragraphs in <div class="p">
- styles adjusted accordingly
- DocBook <para> cannot be nested into <para> - replaced with <informalexample>
- remove empty <div class='xsdoc'/> because they mess output

    There is no such a template (name="dir") in docbook-xsl-1.62.4

    Is it really required here?

    Please clarify also why <p> should be replaced with <div class="p">? They are both standard...

    (name="dir") template is from docbook-xsl-1.71.1\xhtml\block.xsl
    I am using 1.71.1 templates as they are more up-to-date with more features like code highlightning, so I've just cut'n'paste this block from there for customization.

    As for <p> - if the aim is to generate valid XHTML (as it stated in html headers) then by standard <p> doesn't allow nested <p> while <div> does. http://www.zvon.org/xxl/xhtmlReference/Output/Transitional/el_p.html And it is just the case with nested schema annotations - their <para> elements are rendered as <p> and contained within parent <p> in output.

    Accepted, however <xsl:call-template name="dir"/> is removed as useless to keep compatibility with old docbook templates.
