There should be an error message that a concern with this name already exists.

Well, it is supposable that there are more transports from the same department at one hospital.

This has nothing to do with transports. This is about the creation of new concerns (Ambulanzen) on the Home page.
If you look there you will see a total number of 3 entries called "Diana´s Test 13".
