I had to kill Windows Explorer in Task Manager. Then I started a new explorer
process, but the ClassicShell start button was no longer there. Short of
restarting the computer, how could I have got it back please?

It should start by itself, unless you killed ClassicStartMenu.exe as well.
If you have, then simply start ClassicStartMenu.exe again.
If that doesn't work, log off and back on.

Btw if you are on Vista, if you start Explorer from an elevated Task Manager,
Explorer starts as admin and naturally Classic Shell can't hook into it. Make
sure Explorer isn't running as admin. Windows 7 Task Manager will always run
    Explorer as non-admin.
