[cl-cookbook-contrib] Changelog

Hi!

I've added a CHANGELOG file to the CVS repository. Its contents came
mainly from my memory and the emails I've kept. Please correct me if
there are any errors in it.

I'd suggest that anybody who submits changes to the cookbook now also
writes a few corresponding lines into the CHANGELOG file.
