#16 Separate index file (chi)

Some reference manuals (e.g. the DirectX manual) has the topic list
in a separate file named .chi. An example of this can be found at
http://albin.abo.fi/~mstorsjo/dx-docs.zip.

At the moment, only the first page is shown when opening these
manuals, and only pages linked from the first one can be opened.
