Add a new include file to replace some of the (Windows CE) system dependent
definitions that used to be in these files. This is one of the differences
between the trunk and this branch.

These are basically the fixes that Eric House sent to MinGW,
the URL for that is in one of the include files in comment,
and the documentation is now in docs/microsoft.

Move stdcall / cdecl back to the compiler as indicated on the list. This also changes the stdcall define so it doesn't spit a warning every time.
