E.g. select s.tail41.i from havoc.g and ray-trace it with "ae 330 0" perspective.
I could reproduce the bug on Windows and Linux with BRL-CAD's versions 7.6.4, 7.18.2 and the 7.18.3 developer version.
Easily reproduced here for pretty much any "ae * 0+-3" so clearly something is screwy with rays coming in perpendicular to the tgc vector.
Attached image shows ae 300 1 on white where you can see the root solver failures just starting on the top-grazing edge and around the circular end-cap on the right.
